It could refer to Iași in Romania, which according to Wikipedia is/was named "Jassy, Jasch, Jassenmarkt, Jasmarkt" in German. I am not sure though, as the certificate is in Polish and Iași is rather far away from Poland. There are also several places currently named Jasień, some of which are in the formerly Austrian part of southern Poland)

The titles of the first two large columns are largely missing, but it's safe to assume that the first one records the birth date and the second one either the date of baptism or of inscription in the birth registry.
